This ticket features one (1) game play. The player gently removes the latex covering the play area to reveal six (6) WINNING NUMBERS and fifteen (15) YOUR NUMBERS, with a prize amount below each of the YOUR NUMBERS play symbols. If any of the WINNING NUMBERS match any of the YOUR NUMBERS, the player wins the prize amount shown below the matched number(s). If a “ ” symbol is revealed, the player wins the prize shown automatically.

How to Claim $1,000 or “$5,000 A Week For Life” Prizes!
When you win a $1,000 or “$5,000 A WEEK FOR LIFE” prize, first sign your name on the back of the ticket, then fill out a Claim Form Mailer at any New Jersey Lottery Retailer location and mail the original Claim Form Mailer, along with the ticket, to Lottery Headquarters. Winners of the “$5,000 A WEEK FOR LIFE” prize must provide a valid birth certificate for purposes of determining the winner's age. Following validation of the ticket, the “$5,000 A WEEK FOR LIFE” prize (guaranteed minimum prize: $5,000,000) will be issued quarterly in advance starting on or about the anniversary date of the claim and will continue for the life of the winner. Neither the “$5,000 A WEEK FOR LIFE” prize nor any benefits attached thereto will be paid to any person under the age of eighteen (18) years. Rather, payment of the “$5,000 A WEEK FOR LIFE” prize will be deferred until the winner has attained eighteen (18) years of age.
