When you win $4,500, $22,500 or $30,000, first sign your name on the back of the ticket. Then fill out a Claim Form Mailer available at any New Jersey Lottery Retailer . Mail the filled out Claim Form Mailer along with the signed ticket to the New Jersey Lottery Headquarters address printed on the mailer. A check will be mailed to the winner following validation of the ticket.
