This ticket will feature one (1) game play: a “Key Symbol Match” play area. The player gently removes the latex covering the play area to reveal three (3) ULTIMATE NUMBERS and ten (10) YOUR NUMBERS, with a PRIZE amount below each of the YOUR NUMBERS play symbols. If any of YOUR NUMBERS match any of the ULTIMATE NUMBERS, the player wins the PRIZE amount shown below the matched number(s). If a “football” symbol is revealed in any YOUR NUMBERS play spot, the player wins a trip to a major sporting event for two people.

When you win an “ULTIMATE SPORTS TRAVEL PACKAGE” or $25,000 prize, first sign your name on the back of the ticket, then fill out a Claim Form Mailer at any New Jersey Lottery Retailer location and mail the original Claim Form Mailer, along with the ticket, to Lottery Headquarters. A check will be mailed to the winner following validation of the ticket.
