When you win $1,000, $2,000 or $50,000, first sign your name on the back of the ticket. Then fill out a Claim Form Mailer available at any New Jersey Lottery Retailer. Mail the filled out Claim Form Mailer along with the signed ticket to the New Jersey Lottery Headquarters address printed on the mailer. A check will be mailed to the winner following validation of the ticket.
